Return the index filename relative to the given host and https state
If hostname and/or https aren't supplied, they will be determined automatically
Examples of return value:
- index.html - HTML cache file
- https.html - HTML cache file for HTTPS used only when HTTPS and HTTP are cached separately.
- index.xml - File extension represents the content-type, in this case XML
- processwire_com-index.html - HTML cache file when hosts cached separately for processwire.com
- www_processwire_com-index.html - Same as above, but for www.processwire.com hostname
